Activity Request Form

The purpose of this form is to ensure that any and all people who need to be aware or prepared for an event will have ADVANCED notice. You will fill this out if your class/team/PTA is scheduling a special event, a field trip, a guest speaker, a school-wide event, etc.

This MUST be pre-approved by admin before you move forward to finalize plans. You will receive a return email once this has been added to the Master Calendar.

Acknowledgement

Acknowledgement I acknowledge that this form must be submitted at least two weeks prior to my requested date, and that approval is subject to calendar availability.
